Health at Home by Baptist Health has been one of Arkansas's leading home health agencies for over 30 years. Our service area covers a large part of Arkansas, spanning a 50-mile radius around our Little Rock and Arkadelphia locations and a 35-mile radius around our Heber Springs location. We accept referrals based on the zip code of your patient’s residence and welcome the opportunity to serve as a home care provider for your patients.

Refer by Phone
We will need a physician’s order, patient demographics, history, physical and medication list faxed to (501) 202-7706 or 1-800-992-6448. If your patient has Medicare or Medicare Advantage, please send a copy of your office/clinic note within the last 90 days, which discusses the same diagnosis or conditions for which home health is being ordered.
Refer by Fax Fax the following to 501-202-7706 or toll-free to 800-992-6448
Patient demographics, insurance information, order for home health signed by the physician/ APRN/ PA, and the most recent physician office note with medication that is signed (electronic or by hand) by the physician/ APRN/ PA with the reason for which home health is ordered.
